Family Practice Patients

We have family physicians who provide full-service primary care to patients of all ages, from birth to elderly. Family physicians provide routine well-baby visits, infant and childhood immunizations. We see children and teens for routine health concerns, acute medical illness or injury, and behavioural and mental health issues.

We provide preventive care and screening for men and women according to up-to-date guidelines and recommendations. Well-woman visits and early prenatal care are provided by all our physicians, however, not all our physicians deliver babies. Our family physicians also provide counselling to patients experiencing acute or chronic stressors and mental health concerns.

** Please read our COVID-19 FAQ page for further information, and recent updates surrounding the COVID19 vaccination**

Due to the COVID-19 pandemic, we have implemented telehealth options for all patients (video conferencing and phone calls) to connect with your family physician. We are also seeing patients in-person at the office, for any and all visits which are deemed necessary to be done in-person.

Please call in to the office if you have ANY questions or concerns about your health, even if you are not intending to physically come into the office.

Most visits will be booked as a telehealth visit first. After your Telehealth visit, your physician will determine if you need to be seen in the office and arrange this accordingly. We will and are continuing to see patients in the office if a face-to-face appointment is necessary.

Please note that your family doctor may have different clinic hours than usual. For urgent matters, our staff may also ask if you are willing to see a different family physician within our office who may be able to accommodate an earlier appointment.

Please review the Consent to Use Electronic Communications (https://centromedical.ca/wp-content/uploads/2020/04/Centro_ecomm_consent.pdf) and our Privacy Policy (https://centromedical.ca/wp-content/uploads/2020/03/patient_handout_-_privacy_of_your_personal_health_information.pdf). ** Please complete this consent form and follow the instructions to send the form back to Centro Medical if your care might require communication via e-mail or other forms of electronic communications. Our staff cannot send you any medically related information electronically unless we have your agreement. DO NOT e-mail your questions please. All questions are addressed by phone only due to confidentiality and limitation in human resources. **

In the case that you do need to come in, we request that patients attend the visit on their own in order to minimize the volume of people in our clinic, except in cases where a companion is necessary for medical assistance or for translation. We ask that all other companions (spouses, friends, parents, etc) remain outside the clinic (in the car if possible, to reduce crowding and to minimize social contact). For pediatric patients, one parent may come in with the child.

We kindly request that you wait outside the office or inside your car upon arrival for your appointment, and call the office to notify the staff that you have arrived. The staff will call you when a clinic room is available for you. Please use hand sanitizer as you enter the clinic and wash your hands frequently. Medical masks are mandatory for all patients and visitors to the clinic, age 2 years and up.  If you do not have your own medical-grade mask, one will be provided for you upon your arrival at the office.

Please continue to be mindful of strict physical distancing measures to slow and limit the spread of infection. Wear a mask, practice meticulous hand hygiene and please stay home if you are ill.

Thank you for your patience, understanding and kindness during this time.
